- List appropriate department name (specific staff person if appropriate - limit to 1 person only).
- Don’t make people think: Avoid multiple email addresses and staff names. 1 email address only, we do the rest (redirect to appropriate staff person).
- CMS forms: Include the estimated turnaround time; Be concise; Limit form options to essential info only.
- In most cases, the following contact information should always be provided together:
- Department name/Contact Person
- Email Address/alias*
- Phone # & extension
*avoid using email addresses/Aliases based on vendor/software (eg. libiii@brocku.ca)
